Activation and Termination of the Emergency Response Plan 
I.  Activation
Activation generally begins with the recognition of the potential for; or    
an actual disaster.

A.  Internal Disaster (Basic)

1.  Upon discovery of a situation / problem/ incident or emergency  immediately notify the operator at extension 3333 with an  explanation of the situation or the Code word for a specific disaster. (The exception is in the event of a fire; you should pull an alarm pull station.) 

2.  The in-house operator will follow the appropriate (appendix) protocol depending on the actual incident / disaster occurring.

3.  To respond to and internal disaster, locate the appropriate Appendix within this overall Plan and respond according to the instruction. 

4.  If the disaster is so intense that it is necessary to fully implement the Hospital Incident Command System, a command post or “Emergency Operations Center” will be established.   

B.  External Disaster 

1.  Activation of a Code 3, will generally  be by the Emergency Room Staff discovering or being notified of an incident which has the need for additional staffing to adequately handle the expected number of patients.  The in-house operator should be notified immediately. 

2.  The in-house operator will contact the Nursing Shift Supervisor who will immediately assess the situation and make the determination to implement Appendix 4 (External Disaster Plan Code-3) of this ERP. 

1.  If it is necessary to implement the External Disaster Plan, the nursing shift supervisor will advise the in-house operator to overhead page a Code-3 and follow the other instructions listed in Appendix 4.
 
2.  Other staff hearing the overhead page should react as designated by Appendix 4 of this policy.

3.  If the disaster is so intense that it is necessary to fully implement the Hospital Command System, a command post or “Emergency Operations Center” will be established.

II.  Administrative Communications

1.  The On Call Administrator shall be contacted in the event of an unusual or emergency situation in-which other that normal operations will occur, or when there is a likely hood of media attention.  The Nurse Shift Supervisor shall contact the on call administrator to give preliminary details and to get instructions on what actions to take.

III.  HEICS

1.  Hospital Emergency Incident Command System is activated for  each Disaster.  The breadth and depth of the activation depends upon the necessity created by the disaster.

V.  Termination

1.  Termination of any disaster response will be the use of the code word  in conjunction with “has been terminated.” (i.e. “The Black Alert has been terminated.”)
